How do you autopopulate a second <select> element with options using AJAX?

I know that I need to use the onChange event from the first element to trigger the AJAX call to query the database and return results, but I'm unclear on how to use PHP and Javascript to create the new options.
I have seen an example before
Say you have the list of option in a db table "options", with fields "optval", "opttxt", where optval is the reference you pass (ie "opt1"), and opttxt, the text to display (ie "A Volvo")
